The Chicago Department of Public Health (CDPH) has released a Request for Proposals (RFP) for: Population health survey to collect reliable, timely and representative data on root causes, health behaviors and health outcomes from adults living in the City of Chicago to enable CDPH and other organizations to use the data to describe and take action towards improving health and health equity. Aligned with the Healthy Chicago 2.0, this program supports: communities in describing and advocating for their health and wellness, understanding health inequities among demographic groups and communities, monitoring trends over time, and informing and monitoring aspects of Chicago’s community health improvement plan.
