Request for Proposals - Family Connects Chicago Pilot: Regional Community Alignment Councils

The Chicago Department of Public Health (CDPH) has released a Request for Proposals (RFP) for: Family Connects Chicago Pilot: Regional Community Alignment Councils.  Aligned with Healthy Chicago 2.0, this program supports: Access to care and support for mothers and infants by ensuring families have access to resources necessary to support appropriate health and development for infants and young children and supporting a city with strong communities and collaborative stakeholders, where all residents enjoy equitable access to resources, opportunities and environments that maximize their health and well-being.

Deadline for Questions:  Wednesday, January 22, 2020

Proposal Due:  Friday, February 7, 2020

Contract Start Date:  Wednesday, January 1, 2020
