Senior Service Caregiver Respite RFP

DFSS is seeking proposals from qualified home-care agencies, hospitals, or facility-based agencies to directly provide in-home Caregiver Respite Services, facility-based Caregiver Respite Services and/or coordination and scheduling of the in-home or facility-based services for clients needing supervision to relieve their caregivers. Caregiver Respite Services are defined as the provision of temporary or regular, short-term substitute care or supervision on behalf of the caregiver for frail and functionally impaired persons aged 60 and over.

A NOTE ABOUT THE CITY'S NEW EPROCUREMENT SYSTEM:

To complete an application for this RFP, ALL POTENTIAL RESPONDENTS will need to set up an account in the eProcurement/iSupplier system.

New Vendors – Must register at www.cityofchicago.org/eProcurement

Existing Vendors – Must request an iSupplier invitation via email.  Include your Complete Company Name and City of Chicago Vendor/Supplier Number (found on the front page of your contract) in your email to eprocsupport@cityofchicago.org.You will then receive a response from DPS so you can complete the registration process.

Registration in iSupplier is the first step to ensuring your agency’s ability to conduct business with the City of Chicago and DFSS. Please allow five to seven days for your registration to be processed.

Proposals must be submitted via on-line application.

E-mailed or faxed proposals will not be accepted.

A Pre-Proposal Webinar will be held on January 25, 2019 from 10:30 a.m. - 12:00 p.m. noon

The purpose of these pre-submittal events is to clarify the RFP process and the scope of the required services. Attendance is not mandatory, but it is strongly suggested that interested applicants attend.
