Johnson & Johnson Update
Due to recent guidance issued by the U.S. Food and Drug Administration and Centers for Disease Control and Prevention for the Johnson and Johnson vaccine, we are changing the type of vaccine offered at Chicago State University to Pfizer, effective immediately. The Pfizer vaccine is a two dose vaccine; to ensure full effectiveness you will need to return for a second dose, three weeks after your first dose appointment. No Onsite Registration
There is NO ONSITE REGISTRATION. You must have an appointment.
Initially, appointments at Chicago State University are limited to specific zip codes: 60617, 60619, 60620, 60621, 60628, 60633, 60636, 60643, 60649, 60655, and 60827 only. The City is working with community-based organizations and Aldermen to directly schedule appointments for these communities. Chicagoans in these zip codes can also register online or call 312.746.4835. You must live in one of the targeted zip codes, otherwise your appointment will be cancelled.
In mid-April, a limited number of appointments will also be available on Zocdoc and through the City’s call center for all Chicago residents in 1a, 1b, and 1c 18+.
Proof of address is required for all appointments. Please see the list of documents one can provide to prove a Chicago address.
The Chicago State University vaccination site is open to Chicago residents 18+ in phases 1a, 1b, and 1c in the following zip codes: 60617, 60619, 60620, 60621, 60628, 60633, 60636, 60643, 60649, 60655, and 60827.

Only those 18+ years of age permitted into the vaccination site. There is NO ONSITE REGISTRATION.
Anticipate spending about 1 hour at the vaccination site. This includes the 15-30 minute observation period. Please wear short sleeves or loose fitting clothing so we can easily administer the vaccine. We recommend you visit the washroom prior to checking-in and bring your own water and/or snack if you will need it. To expedite the onsite process, bring a copy (print or digital) of your registration email to your vaccination appointment.
