The Health Promotion Services for Older Adults RFP seeks qualified community-based health organizations interested in providing health promotion services to older adults, aged 55 and older, at our network of 21 city-wide senior centers, consisting of six regional senior centers and 15 satellite senior centers, in addition to online and telephonic program offerings.

The goals of the Health Promotion Services for Older Adults RFP are to assist older adults to:

  • Increase health awareness
  • Increase self-determination in managing their health
  • Increase positive health behaviors

 

The DFSS Senior Services Division has provided health promotion services to older adults for over two decades. The program, delivered by qualified health professionals at our 21 senior centers and through online and over-the-phone platforms, offers the following core services:

  • Health Education (i.e. evidence-based workshops and educational presentations on a wide range of health promotion topics germane for older adults)
  • Health Screenings (i.e. blood pressure checks)
  • One-on-One Consultations (i.e. confidential and personalized support)
  • Mental Health Support Groups (i.e. peer-to-peer support on successful adaptation to life changes and losses)

Social Isolation Programming (i.e. virtual and telephonic health educations classes, wellness discussion groups, brain health activities, and other innovative initiatives.)

In FY2019, 6,404 unduplicated seniors participated in health promotion services and 5,116 units of service (one activity session or hour of related service provision) were performed. In FY2020, amidst the COVID-19 pandemic, 3,669 unduplicated seniors participated in health promotion services (i.e., in-person, over-the-phone, and online) with 2,863 units of service performed. The selected Respondent is expected to assist with outreach efforts to increase the number of older adults accessing services (e.g., reaching out to past participants with information about upcoming programs and promoting programming during lunch at the senior centers and through social media platforms).

 

DFSS anticipates funding a minimum of two delegate awards through this RFP, with an estimated site reimbursement allocation of $11,250 for Satellite center locations and $30,735 for Regional Center locations. This contract will operate on a reimbursement basis only. No advances will be given.

 

Interested applicants can also apply for to provide dietician consulting as an optional program component. The dietician consultant drafts menus and reviews nutrition program standards for the congregate meals and home delivered meals programs.

This comprehensive menu of health promotion services has been designed to empower older adults in remaining healthy and safe within their homes (via online and over-the-phone programming) and communities (via neighborhood senior center programming). In turn, this aligns with our division-level goal of supporting older adults to live independently for as long as possible.
