The Department of Family and Support Services (DFSS) seeks to expand our current Service Coordination and Navigation (SCaN) program model. SCaN is a community-based program model; SCaN providers must be flexible, adaptative, and highly skilled in working with youth, ages 14-24, at the highest risk of violence regardless of gender, sexual orientation ethnicity and race. This RFP seeks to stabilize eligible youth in their current environment, increase their self-efficacy (confidence in the ability to exert control over one’s own motivation, behavior, and social environment), and assist in their transition to adulthood. Ultimately, we hope that these interventions can help reduce their involvement in violence and contribute to a reduction in violence at the community-level. Additionally, organizations will be required to participate in enhanced capacity building (“learning cohort”) that will strengthen their understanding and interaction with the target population and increase their exposure to best practices in violence reduction strategies.

We seek to expand our current SCaN program to include four additional community areas in 2022 – Greater Grand Crossing, New City, South Lawndale, and South Shore. Current SCaN delegate agencies are not eligible to apply in an effort to increase community capacity and ensure place-based knowledge of the targeted community areas.

 

DFSS is looking to support a lead partner in a community area that has:

  • Expertise applying best practices for working with youth, service coordination, and navigation models (see below)
  • Strong and effective communication and networking skills with youth at the highest risk of violence involvement and with referral partners. For example, Chicago Police Department, Cook County Probation and Court Services, Illinois Department of Juvenile Justice.
  • The building blocks for working with youth at the highest risk of violence involvement, but also incorporating youth voice throughout the process
  • A mechanism to provide support for placed-based, community-based organizations to create a service delivery network
  • Experience serving youth with the highest risk of violence involvement successfully with strong performance outcomes
  • Current programming options in one of the four community areas identified on page 10
  • Demonstrated skills creating and implementing a comprehensive outreach strategy and plan.

DFSS anticipates funding four agencies through this RFP. We anticipate that each award will be between $300,000 to $500,000 annually depending on the size of the proposed program. This contract will operate on a reimbursement basis only. No advances will be given.
