DESCRIPTION:  Department of Family and Support Services (DFSS),

[Service Coordination and Navigation for Youth at the Highest Risk of Violence RFP (SCaN)]

 

The Chicago Department of Family and Support Services (DFSS) seeks to fund community-based organizations, referred to as “delegate agencies”, with experience providing intensive community-based case management services to youth ages 14-24 who are at the highest risk for violence in the target community areas South Lawndale (Lower West Side) and Rogers Park (Edgewater, Uptown).

 

 RFP #9755, Service Coordination and Navigation for Youth at the Highest Risk of Violence was issued in November 2023.  The Prior RFP covered 31 community areas and had one (1) submission for South Lawndale/Lower West Side which was not awarded.  In addition, DFSS determined the need to add three additional community areas to ensure we were availing services to as many youth at the highest risk of violence.  Therefore, the Department is issuing the current RFP, which covers only South Lawndale (Lower West Side) and Rogers Park (Edgewater, Uptown). 

 

The term of contract(s) executed under this RFP will be from July 1, 2024– December 31, 2025, with the option of a one-year renewal dependent on the selected Respondent’s performance and the continued availability of funding. DFSS anticipates funding 1-2applications per community area within the range of $222,000 - $444,000 per contract per year. Total available funding is $756,000.
