Re-Entry Support Program Locations
The community re-entry support program is setup to reduce recidivism among the formerly incarcerated by providing a "navigator" that will refer the returning residents to agencies that deliver case management, skill assessments, individual goal development, job-training, housing, substance abuse counseling and other support services.

Chicago is one of the few cities to focus on the challenges of prisoner reentry in such a thorough and comprehensive manner. Who is the CCHR?
The Commission on Human Relations promotes appreciation of Chicago's diversity and works to eliminate prejudice and discrimination. Commissioners, advisory councils, and staff conduct pro-active programs of education, intervention, and constituency building to discourage bigotry and bring people from different groups together.
