The Illinois Freedom of Information Act (FOIA) is a state law that provides access to certain public records. Each City department is a separate agency responsible for maintaining its own records, so requests should be submitted to the department that maintains the records you want.
Police Board records that are accessible under the FOIA include the record of proceedings of Police Board disciplinary cases.

The request must be in writing and include a description of the records that are being requested (please be as specific as possible). You may use this form, although you are not required to do so.
If photocopies of records are requested, a fee of no more than 15 cents per page may be charged for copying in excess of 50 pages to cover reproduction costs.
