DFSS seeks proposals from qualified in-home care agencies who have experience providing assistance with activities of daily living (i.e., bathing, grooming, dressing) or instrumental activities of daily living (i.e., shopping for groceries, housekeeping, taking medication) for older adults 60 years of age or older in the city of Chicago.
The Chore Services program provides temporary in-home assistance to older adults 60 years of age or older who have difficulty with one or more activities of daily living (ADLs) (i.e., personal care) or instrumental activities of daily living (IADLs) (i.e., household tasks). Older adults may have difficulty completing tasks due to physical or cognitive limitations. Chore Services are provided under the supervision of the client or other responsible person. DFSS seeks to procure a delegate agency to deliver services such as housework, laundry, grocery shopping, and meal preparation, assistance with bathing, grooming, or dressing. Individuals needing more heavy-duty chore services can receive assistance with environmental hazards that threaten the health and safety of the older adult, including hoarding or other unsafe conditions
The selected Respondent will be responsible for the program management including outreach, program implementation, and data collection on the impact of programs on older adults served.
