Home Delivered Meals

The City of Chicago’s Home Delivered Meal (HDM) Program provides nutritious meals to eligible Chicago residents, 60 years of age and older, who are frail and/ or homebound because of illness, incapacitating disability, or are otherwise isolated.  An in-home assessment is conducted by a case manager to determine eligibility for this program. Eligible clients receive either a daily delivery of hot meals or a weekly delivery of frozen meals based on the meal diet received. The following meal diets are offered through the Home Delivered Meals Program:

  • General Diet
  • Vegetarian Diet
  • Kosher Diet
  • Pureed Diet
  • South Asian Diet (Indian and Pakistani meal options)
  • East Asian Diet (Blended menu of Chinese, Korean, and Vietnamese meal options)

 

All Home Delivered Meal menus are approved by DFSS’s Registered Dietician and provide 1/3 of the daily nutrition needs of the average older adult. A suggested contribution is requested for all meals; however, no client is denied a meal should they be unable or unwilling to contribute.
