DESCRIPTION: Department of Family and Support Services (DFSS),
[Senior Food Box Program RFP ]
The Department of Family and Support Services (DFSS) seeks proposals from qualified food service providers who have experience providing monthly or quarterly delivery of fresh produce, pantry staples, plant-based foods, and frozen meat options for older adults age 60+. DFSS seeks to provide services citywide to six Regional Senior Centers. Clients may receive monthly or quarterly distribution of senior food boxes and target outcomes will include an overall self-reported decrease in food insecurity.
The term of contract(s) executed under this RFP will be from October 1, 2025 – September 30, 2028. Based on need, availability of funds and contractor performance, DFSS may extend this term for up to [two additional years] with each extension not to exceed one year. Continued support will be dependent upon the selected Respondent’s performance and the continued availability of funding. We anticipate that the annual award will be $250,000 [total $750,000 for three years] depending on the size of their proposed program. DFSS anticipates funding one agency. This contract will operate on a reimbursement basis only. No advances will be given.
This initiative is administered by DFSS through Older Americans Act’ Federal funds. Consequently, all guidelines and requirements of DFSS, the Illinois Department on Aging and the Older Americans Act must be met. Additionally, all delegate agencies must comply with the Single Audit Act if applicable.
