Who We Are

CDPH's Heart Disease Prevention and Treatment programming seeks to reduce heart disease and reduce or eliminate health inequities that contribute to the racial life expectancy gap in Chicago. CDPH focuses on increasing community access to heart disease prevention resources and promoting risk reduction behaviors in priority communities on Chicago's South and West Sides.


What We Do 

CDPH provides heart disease prevention and treatment services through Community Health Workers as part of the Community Health Response Corps in priority areas. The program brings blood pressure screenings, health literacy resources, and culturally appropriate messaging directly to residents in communities most affected by cardiovascular disease.

CDPH's Community Health Response Corps connects residents to local health resources through the Resource Coordination Hub hotline (312-746-4835), which provides referrals to services and information about hypertension management.

CDPH also collaborates with community-based organizations to provide programs and resources that address the social determinants of health influencing cardiovascular disease, including food access, physical activity opportunities, and tobacco cessation support in the Chicago communities with the highest rates of heart disease and related risk factors.
