How to Prepare

Please call our clinics to schedule an appointment.

We ask that patients present two items at registration:

  • Photo ID (such as a school identification card, state identification card, or driver’s license):
If you do not have a photo ID at the time of your visit, you can still get STI/HIV testing and same-day HIV test results. However, you must present photo identification when you return for your other test results.
  • Medical Insurance Card (for those with insurance):
The Chicago Department of Public Health is committed to providing high-quality care to our patients in the STI Specialty Clinics, regardless of your ability to pay or whether you have insurance.

Patients will need to arrive 10 minutes before their scheduled appointment in order to complete registration. If you arrive 15 minutes late for your appointment you may need to reschedule. All patients are required to wear masks that cover both the nose and mouth. Only patients with scheduled appointments are allowed into the clinic.

The Chicago Department of Public Health is working with City, County, and State guidelines to help protect all its constituents' health and wellness during these uncertain times.
